At Synergy Physical Therapy & Sports Medicine we never want the cost of treatment to stand in the way of receiving the care you need to feel great again. We have developed a plan to address the needs for therapy services for those without insurance, who have out of network plans, and those who have exhausted their insurance benefits. With the addition of Care Credit, you can get on the road to recovery for less than $ 100.00 per month.
This program is available as a simple Cash-Pay (cash, check, or credit card) service.
There are no hidden costs or fees, just the quality care that you can expect from Synergy.
The Pain Relief Program includes the following:
- 1 Initial Evaluation and treatment session and up to 5 follow up sessions per case*.
- Client interview regarding musculoskeletal symptoms, previous health/injury history review.
- Hands-on medical screening and evaluation of musculoskeletal injury, determining the source of your symptoms.
- Highly specialized hands-on treatment including use of special medical equipment.
- Progressive exercise and stretching to maximize recovery.
- Tips to maximize individual healing potential.
- Personalized home exercise program.
- **Please Note** some of the specialty treatments like pelvic floor physical therapy may require the purchase of additional items. Speak with your therapist prior to Program purchase.
* One case per body part or region. Additional body parts will require new Evaluation and Plan.
Pain Relief Program - $ 625 (Additional visits can be purchased for $ 90 each)

- Option 1: Single Payment with cash, check, or credit card.
- Option 2: Two equal payments, ½ Prior to the start of your Initial Evaluation, ½ prior to your 4th visit.
