Synergy is excited to announce that we are implementing a new Patient Registration Portal service. This allows patients the ability to complete our registration forms, as well as to submit demographic information from the comfort of their own home. There is no risk associated with this process, it is HIPAA compliant and will import directly to your chart for the Front Office Administrators and Physical Therapists to review. The portal will walk you through step-by-step, upon completion the registration will be sent to our team, at which time our Patient Care Coordinators will contact you to schedule your initial appointment.
If you are unable to complete the registration through our Patient Portal then you would need to arrive 30 minutes prior to the time of your appointment in order to complete it on site.
